What is Sculptra,
and how does it work?
At Skin Clinic Med Spa, Sculptra in Riverside offers something genuinely different from traditional filler: instead of adding volume from a syringe, it prompts your body to rebuild its own.
Sculptra is made from poly-L-lactic acid (PLLA), a biocompatible material used in dissolvable medical sutures for decades. Injected into the deeper layers of the skin, the PLLA microparticles act as a scaffold that signals your fibroblasts to produce fresh type I and type III collagen — the structural proteins that give skin its firmness and support. Over the following months, the particles are absorbed and what remains is your own newly built collagen.
This is why Sculptra behaves so differently. It doesn’t fill a line; it rebuilds the foundation beneath it, restoring the structural support that volume loss takes away. As a result, the change is gradual and remarkably natural — most people notice you look well, without identifying why. Results typically develop over two to three months and can last two years or more.
Every treatment is performed by licensed injectors under the medical direction of Dr. Syed W. Bokhari, MD. Sculptra also works beautifully alongside dermal fillers for immediate definition, and Botox for expression lines — together they address structure, contour and movement.
Where Sculptra
rebuilds structure
Hollow Cheeks
Rebuilds structural support high in the cheek — the single biggest driver of a tired, deflated look.
Temple Hollows
Restores volume to sunken temples, softening a drawn or gaunt appearance around the eyes.
Nasolabial Folds
Softens the folds from nose to mouth by rebuilding the support above them, not just filling them.
Marionette Lines
Lifts the lines running from mouth corners to chin for a softer resting expression.
Jawline Definition
Restores collagen along a softening jawline for a firmer, better-defined lower face.
Overall Skin Laxity
Improves skin thickness and firmness across the whole face — a quality change, not just volume.
Neck & Décolletage
Improves crepey texture and thin skin on the neck and chest, where lines are hard to treat.
Body Areas
Adds gradual, natural volume and smoothing to hip dips and the buttocks without surgery.
Rebuilds structure,
not just volume
Hyaluronic acid filler adds volume exactly where it's placed, immediately. Sculptra works across a broader area, rebuilding the collagen foundation that supports your whole face. That's why it excels at overall firmness and skin quality — and why it can't do precise work like lips.
- Stimulates your own type I and III collagen
- Improves skin thickness and firmness, not just contour
- Treats the whole face rather than one line
- Not suitable for lips or under-eyes — use HA filler there
The massage protocol
is part of the treatment
After each session you'll massage the treated areas for five minutes, five times a day, for five days. This isn't optional aftercare — it distributes the PLLA particles evenly so the collagen your body builds is smooth and uniform. Skipping it is the main cause of nodules.
- 5 minutes, 5 times daily, for 5 days after each session
- Distributes particles evenly for a smooth result
- Significantly reduces the risk of nodules
- We teach you the exact technique before you leave
Patience now,
years of results
Sculptra asks something unusual of patients: trust the process. You'll look full on day one from the carrier fluid, deflate by week two, then improve steadily for months. Patients who understand this timeline are the ones who love their results — which is why we explain it thoroughly before you commit.
- Results build over 2–3 months, peaking around 6
- Longevity of 2 years or more — far beyond HA filler
- Ageing looks gradual, so nobody clocks the change
- Annual maintenance keeps the result topped up
The treatment process
step by step
Consultation & treatment plan
We assess your facial structure and degree of volume loss, then map out how many vials and sessions you'll realistically need — usually two to three, spaced four to six weeks apart.
Reconstitution & preparation
Each Sculptra vial is reconstituted with sterile water well in advance of your appointment, which is essential for an even, smooth result.
Numb & prepare
We cleanse the area and apply topical numbing. The Sculptra itself is mixed with lidocaine, so the treatment stays comfortable throughout.
Deep, layered injection
Using a needle or blunt-tipped cannula, your injector places the product in the deep dermal layer across the treatment area — building a scaffold rather than filling a single spot.
Massage & your 5-5-5 plan
We massage the area thoroughly, then teach you the at-home massage protocol. This part isn't optional — it's how your results end up smooth and even.
Your Sculptra journey,
month by month
The temporary 'deflation' in week two catches everyone off guard. It's expected — Sculptra is playing a long game, and the result is worth the wait.
How to prep & care
including the 5-5-5 rule
Before your appointment
- Avoid blood thinners, fish oil and vitamin E for 5–7 days (with your doctor's approval)

- Let us know if you're prone to cold sores or have had filler complications
- Avoid dental work or vaccinations within two weeks either side
- Reschedule if you have an active infection or breakout at the site
- Plan ahead — allow at least three months before a major event
After your appointment
- Massage each treated area 5 minutes, 5 times a day, for 5 days — this is essential
- Use fingertip pads with moderate pressure in small circular motions
- Apply a cool compress in short intervals for swelling in the first 24 hours
- Skip strenuous exercise, saunas and hot yoga for 24–48 hours
- Sleep slightly elevated on the first night
- Some providers extend gentle massage for another 1–2 weeks — we'll advise you

Sculptra with filler
and tox
These treatments solve different problems, and the best results usually combine them. Sculptra rebuilds the collagen foundation, dermal filler adds immediate contour and definition where precision matters, and Botox softens expression lines. We'll sequence them properly across your visits — ask us about combining at your consultation.

Skin Clinic for Sculptra
Proper preparation matters
Sculptra is reconstituted well ahead of your appointment and injected at the correct depth. Rushed preparation and shallow placement are what cause nodules.
Physician-supervised
Care is overseen by our Medical Director, Dr. Syed W. Bokhari, MD — and we screen carefully for the autoimmune and scarring histories that make Sculptra unsuitable.
Honest about the timeline
We'll explain the week-two 'deflation' before you book, not after you panic. Sculptra suits patients who want gradual change — and we'll tell you if that isn't you.

Which is right for you?

Filler adds volume immediately using hyaluronic acid and lasts 6–18 months. Sculptra is a collagen biostimulator — it prompts your own body to build collagen gradually over two to three months, with results lasting two years or more. For example, filler is ideal for lips; Sculptra is ideal for overall facial structure and firmness.
You'll look fuller on day one, but that's the sterile water carrier and it fades within a week or two. Real results begin appearing around month one to two and reach their peak at three to six months as new collagen forms.
It's the Sculptra massage protocol: massage each treated area for five minutes, five times a day, for five days after each session. This distributes the particles evenly so your collagen builds smoothly, and significantly reduces the risk of nodules. It's a genuine part of the treatment, not optional aftercare.
Two years or more for most patients — considerably longer than hyaluronic acid filler. Because it builds your own collagen, the result fades very gradually. Many patients have one or two maintenance vials annually to preserve it.
No, and this is an important difference. Unlike HA fillers, Sculptra can't be reversed with hyaluronidase because it stimulates your own tissue rather than sitting in it. That's precisely why careful injector selection, correct depth and proper technique matter so much.
We apply topical numbing and the product contains lidocaine, so it's comfortable. Expect some swelling, tenderness and possible bruising for a few days. You can return to normal activities right away, though avoid strenuous exercise for 24–48 hours.
Sculptra isn't ideal if you want instant results, need precise lip or under-eye correction, or aren't willing to commit to the massage protocol. We also screen carefully for autoimmune conditions, a history of keloid scarring and active skin infections. We'll tell you honestly if another treatment suits you better.
What our patients say
Sculptra patients say
“Jessica warned me I'd panic at week two and she was completely right — then around month three my cheeks looked incredible. Nobody can tell I've had anything done, they just say I look well.”
“I'd had filler before and always felt slightly overdone. Sculptra was the opposite — gradual, natural, and my skin actually feels firmer, not just fuller. Two years on and it's still holding.”
“They explained the 5-5-5 massage properly and even had me practice it before I left. No lumps, beautiful even result. That attention to detail is why I keep coming back.”
